Assumption of Risk

I understand that participation in arts and crafts activities may involve certain risks, including, but not limited to, minor cuts, scrapes, burns, skin irritation, allergic reactions, spills, slips, falls, or injuries associated with the use of craft supplies, tools, adhesives, scissors, hot glue guns, paint, and other materials.

I understand that Kay Kraft Club provides age-appropriate activities, materials, instructions, and safety guidance; however, not all risks associated with participation can be eliminated.

I voluntarily choose to participate, or allow my minor child to participate, and knowingly assume the ordinary risks associated with the activity.

Craft Tools & Materials

I understand that KKC activities may involve craft tools and materials that require care and proper use.

I agree that I, or the minor participant for whom I am responsible, will follow instructions and safety guidelines provided by Kay Kraft Club.

I understand that KKC may restrict a participant’s use of a particular tool or material when necessary for safety and may provide assistance or an alternative when appropriate.

Allergies & Sensitivities

I understand that craft activities may involve materials such as adhesives, paints, fibers, wood, foam, fabric, yarn, latex-containing products, or other craft supplies.

I am responsible for notifying Kay Kraft Club before participation of any known allergy or sensitivity that may be relevant to the activity.

I understand that Kay Kraft Club cannot guarantee an allergen-free environment.

Minor Participants & Caregiver Supervision

For classes or activities requiring an accompanying parent or caregiver, including Little Krafters, I understand that the accompanying adult must remain present and is responsible for actively supervising and assisting their child throughout the activity.

For other youth classes where parent/caregiver attendance is not required, I authorize my child to participate under the supervision and instruction of Kay Kraft Club for the duration of the scheduled class.

Personal Property

I understand that Kay Kraft Club is not responsible for lost, stolen, misplaced, or damaged personal belongings brought to a class, workshop, program, or event.

Emergency Situations

In the event of an emergency involving a minor participant and I cannot immediately be reached, I authorize Kay Kraft Club and its instructors or assistants to contact emergency medical services when reasonably believed necessary for the participant’s health or safety.

I understand that I am responsible for any medical expenses resulting from emergency care or treatment.

Release of Liability

To the fullest extent permitted by Arizona law, I voluntarily release and hold harmless Kay Kraft Club LLC, its owner, instructors, assistants, representatives, and agents from claims arising from the ordinary and inherent risks associated with voluntary participation in Kay Kraft Club activities, except to the extent a claim cannot legally be waived or released.

I understand that this agreement does not waive rights or claims that cannot lawfully be waived under applicable law.

Participant Conduct

I understand that Kay Kraft Club may stop or limit a participant’s involvement in an activity if the participant is using materials or equipment unsafely or behaving in a way that creates a safety risk for themselves or others.

Acknowledgment & Agreement

By selecting the required waiver acknowledgment during the Kay Kraft Club booking process, I confirm that:

• I have read and understand this Participant Waiver & Release of Liability.

• I have had the opportunity to review the information before agreeing.

• I voluntarily agree to these terms.

• If agreeing on behalf of a minor, I represent that I am the minor participant’s parent or legal guardian and have authority to agree on their behalf.
